To obtain a U.S. passport, you must provide the following:
- Application (can be found online or at the Clerk’s Office)
- 2” x 2” passport photo (photos are available at the Clerk’s Office for $15)
- Proof of citizenship (certified birth certificate, naturalization certificate, valid or expired U.S. passport)
- Valid driver’s license or state ID (identification)
- Check or money order
- Note: For anyone under the age of 16, both parents must be present
